Appointments
Office hours: 9 am to 5 pm, Mon to Fri
Appointments must be scheduled.
You will need to complete an information packet prior to your first appointment. We will supply you with the forms at your first visit. Please come 15 to 30 minutes early for your first visit so that you will have time to fill in the forms. Periodically we will ask you to udate the same forms. We know that may seem inconvienent, but it is necessary for us to keep current on any changes to your health, medications or insurance information.
If you prefer you can fill out the Information forms online and print them out at home and bring them in with you or fax them to our office. Please follow the link for Office Forms.
Emergencies
If you have a serious emergency after hours, please go to the hospital emergency room and ask the doctors there to notify me.
Phone Calls
After normal business hours you can leave a message on our voice mail which will be answered on the next business day.
Prescription Refills
Prescription refills will only be handled during our normal business hours
