Fees and Insurance

December 29th, 2008

Your health insurance is an agreement between you and your insurance carrier to pay for medical care. You are responsible for any unpaid balances and for confirming your insurance coverage. Any services not covered by your insurance provider will be your responsibility.

Urology Care does not participate with all insurance companies, but is always willing to work to help you in the event your insurance does not cover the full fee. You have every right to see physicians out of your insurance companies network.

Co-payments

Any co-payments and balances are to be paid at the time of visit.

Non-Participating/Out of Network

Even if we do not participate with your insurance company, you can still come and see us. Most insurance companies will allow you to see physicians out of their network. They will pay the physician an out of network fee which is commonly about 10-20% less than they would normally reimburse us. You will be billed for the balance
